Which Opioids?

Weak Opioid
  • Codeine, dihydrocodeine
Partial Opioids
  • Tramadol, Buprenorphine
Full Opioids 
  •  Morphine, Oxycodone, Fentanyl, Methadone, Pethidine 

Indications for use of opioids

Acute Pain
  • Post operative pain, trauma pain etc.
  • Use in acute severe, nociceptive, non-cancer pain
  • Short duration of pain - use a short acting drug
Cancer pain 
  • Acute/chronic/acute on chronic, nociceptive, cancer pain
  • Long duration of pain - use a long acting drug
  • Incident pain (e.g. on movement) - use a short acting drug
Chronic Non-cancer Pain
  • There is no role for long term opioids in chronic non-cancer pain

Neuropathic Pain

  • Role in acute, severe neuropathic pain - may be of some benefit??
  • No role in chronic, non-cancer neuropathic pain
