Opioids in Acute Pain - for example post operative pain


Morphine is the Gold Standard and should be used unless there are any contra-indications

Check local guidelines/protocols!!

Morphine as per IV Protocol
  • 1-2mg every 5 minutes under direct supervision
Morphine oral route 
  • Immediate release-Sevredol
  • 10-20mg 1 hourly PO PRN
Fentanyl
  • "2nd line" IV opioid - alternative if unable to use morphine
  • Fentanyl as per IV protocol
  • 10-20mcg every 5 minutes under direct supervision
Oxycodone
  • "2nd line" oral opioid - alternative if unable to use morphine
  • Oxycodone immediate release-Oxynorm 
  • 5-10mg 2 hourly PO PRN
  • Oxycodone as per IV protocol (sometimes prescribed by Acute Pain Service)
    • 1-2mg every 5 minutes under direct supervision 
Tramadol
  • Immediate release Tramadol 50-100mg PO QID PRN
  • Used in conjunction with other analgesics
